How does the Local Link Program work?
The Local Link connects primary healthcare providers and patients with a domestic, family and sexual violence specialist, offering the following services:
- Safety planning and risk assessment tailored to each patient’s situation
- Connection to appropriate support services through streamlined referral pathways for domestic, family, sexual violence, and child sexual abuse
- Telephone advice and support for clinicians including secondary and joint patient consultations to assist clinicians and patients in primary care settings
- Feedback on referral outcomes to support continuity of care in your practice
